Apply for ANZ Visa Purchasing

To apply for an ANZ Visa Purchasing Card:

1. Download and read the >ANZ Commercial Cards Terms and Conditions (PDF, 448kb).
   
   
2. Print the forms below, complete the details and fax them to 1800 459 143.

Account Set-up Form

This form is required to set-up your overall Business Credit Card account.

Cardholder Registration

This form is required to establish each cardholder. If you require more than one cardholder please complete as many forms as necessary.

CardPay Direct (to set-up automatic payments - Optional)

This is the most convenient method of payment.

This authorises automatic payment (direct debit) of the Billing Account(s) on the 'Payment Due Date' from the company's nominated bank or building society account. Please check with your bank or building society before applying for CardPay Direct, as this is not available from some accounts.

Print the >CardPay Direct Application form (PDF, 192kb), complete the required details and return with the mandatory forms.
