Apply for ANZ Corporate Card
To apply for a new ANZ Corporate Card there are mandatory forms and optional forms to complete.

A. Mandatory forms
To apply for an ANZ Corporate Card:
- Download and read the ANZ Commercial Cards Terms and Conditions (PDF, 448kb).
- Download and complete the Setting up a New Commercial Card Facility form (PDF, 112kb).
- Download and complete the Cardholder Registration form (PDF, 156kb).
B. Optional forms
The following forms are only required if you wish to have the functionality set on your ANZ Corporate Card.
This is the most convenient method of payment.
This authorises automatic payment (direct debit) of the Billing Account(s) on the 'Payment Due Date' from the company's nominated bank or building society account. Please check with your bank or building society before applying for CardPay Direct, as this is not available from some accounts.
Print the CardPay Direct Application form (PDF, 192kb), complete the required details and return with the mandatory forms.
topMerchant Category Group Controls (MCG)
This option allows you to restrict some or all cardholders from purchasing goods and services from entire Merchant Groups eg. MCG 0640-Air and Space Transport (blocking all airlines) or MCG 0571-Accommodation (blocking all hotels, motels and resorts etc.).
Print the Merchant Category Group Controls form (PDF, 59kb), complete the required details and return with the mandatory forms.
Merchant Category Code Controls
This option allows you to restrict some or all Cardholders from purchasing goods and services at particular Merchants Category's without blocking the entire Merchant Group eg leave MCG 0523-Furniture, Houseware and Appliance Retail unblocked on the MCG form, but block the MCC 5251-Hardware stores on this form. All other MCC's within the MCG would then continue to be authorised and only purchases at hardware stores would be declined.
Print the Merchant Category Code Controls form (PDF, 812kb), complete the required details and return with the mandatory forms.
